Prepare the Chicken and Vegetables
- Place the chicken breasts in the crockpot.
- Add the sliced onion, green bell pepper, red bell pepper, minced garlic, Italian seasoning, salt, and black pepper on top of the chicken.
Cook the Mixture
- Cover and cook on low for 6 hours or on high for 3 hours until the chicken is cooked through and tender.
Make the Cheese Sauce
- In a bowl, mix the softened cream cheese, shredded mozzarella cheese, and chicken broth until smooth.
Combine and Serve
- Once the chicken is cooked, shred it with two forks and stir in the cheese sauce until well combined.
- Serve the mixture in whole grain sub rolls.
